Case study 02

JISR AI. A clinical workspace for outpatient clinics.

The patient completes her history on her phone. The doctor enters with a structured chart already prepared. Obstetrics and gynecology was the first protocol. A new specialty means new questions and a new report, not a new system.

Sector
Outpatient clinics
Type
EMR with a pre-visit interview
Role
End-to-end owner: data model, interview engine, AI, doctor report, mobile patient experience, and clinic board

The business problem

The first part of the visit was spent taking history on paper or by spoken questions. Detail changed with who asked, how much time there was, and how the patient could describe what she feels.

  • The complaint appears while the patient is already in the room.
  • Everyday answers are recorded incomplete and get lost between reception and the consultation.
  • A returning patient answers the same questions again. Empty fields were sometimes filled with assumptions.
  • The appointment, the interview link, and the report were not connected.
  • What was missing was a stable way to turn any specialty protocol into a history collected before the visit.

What the platform changes

Clinic measurements are not listed. Worth measuring later: minutes of history-taking inside the visit, charts ready before the appointment, and flags that reached the doctor before the patient came in.

Before the visit

The patient completes the interview from a link and a QR code, in Arabic or English. Questions can be read aloud.

The path follows the answers

The route changes with the answers and the visit type. It is not one questionnaire for everyone.

A chart in the shape of the report

Clear sections, flags with the reason they appeared, suggested investigations, and a short case presentation written doctor to doctor.

AI gathers and presents

It does not diagnose and it does not prescribe. The structured report still appears if the narrative is not generated.

Returning patients

Only data already in the record is loaded. Every old value shows its source. An empty field stays empty.

The clinic board

Appointments, interview sessions, and report-review status sit together, so the doctor knows who is ready.

Limits of the model

  • The presentation is built from fields that were asked and answered, the clinical term, the flags, and suggested investigations. The raw chart is not sent to the model.
  • Numbers and dates, including Arabic numerals and approximate speech, are read in the engine before any model call.
  • Going back removes that answer’s effect from the session and the report.

System workflow

No names, phone numbers, or file numbers. Obstetrics and gynecology is the first specialty on this engine, not the limit of the product.

The patient finishes the history before the consultation. The same path runs for another specialty by changing the clinical configuration.

Technical architecture

FrontendNext.js 16, React 19, TypeScript, Tailwind CSS 4, Arabic RTL and English
RecordPostgreSQL and Prisma: visits, labs, allergies, medications, appointments, pre-visit sessions, and prescriptions
InterviewA fixed engine. Specialty logic is a configuration file: questions, routing, calculations, flags, and report sections
ModelOpenAI for classification, follow-up outside ready classes, a clinical term, and the case presentation
VoiceElevenLabs, with a live voice for a question that is not recorded yet

Testing and ownership

  • Automated scenarios cover classification, dates, unclear answers, going back, and loading a previous visit. The first set is the obstetrics and gynecology protocol.
  • The protocol was built from that specialty’s clinical-logic document and tied to the patient record and the clinic path.
  • Two roles: the doctor reviews reports. Reception manages charts, appointments, and the interview link.
